A Case of Septic Cavernous Sinus Thrombophlebitis Presenting with Bilateral Abducens Nerve Palsy

Abstract

|
We report a 43-year-old man who presented with bilateral horizontal diplopia due to bilateral CN 6th palsy, and fever, erythema multiforme in the lower extremity. Brain MRI and CSF study suggested a bacterial meningoencephaitis, and chest radiography revealed bilateral multiple pulmonary nodules. Blood culture was positive for Streptococcus constellatus. Septic cavernous sinus thrombosis associated with multiple pulmonary septic emboli was diagnosed. After intravenous antimicrobial therapy and anticoagulation, he was recovered completely without ocular sequelae.
